Please put your date of birth in your message. This online form is for NON-EMERGENCY questions only. If you have a medical emergency, please call the office, dial 911, or go to the nearest emergency room.

By submitting an inquiry through this website, you authorize Orthopedic Associates of Dutchess County to respond via email.

NOTE: You may need to complete a HIPAA Medical Records Release form in order for your records to be given to you. This form can be found in the link below. Print and complete, then bring to our office or fax it to 845-471-7888 or mail it to our Poughkeepsie office (1910 South Road, Poughkeepsie, NY 12601).
