The easiest way to set up an account is by requesting an email invitation from us. Please provide the correct email address to one of our patient representatives, and we will send you an invite.

Patients can also create their account without an invite. At the top of the page click on Patient Portal. Once you are on the portal page select “I need to sign up.” Complete the requested info, then you will be able to request a connection to Orthopedic Associates of Dutchess County during this process. We then will verify their account and connect your medical records.

Yes we do. You can download the FollowMyHealth® Universal health record App to view your information from your Device.

For any Demographics or insurance changes we do get an electronic notification and we will update the information in our system.

**When the patient add an item to their universal health record, it gets stored online only. If they would like to update or change any medical information patient must compose and send a message to their provider. Our medical records Team would review it and send it the appropriate Department**.
